1. Primary Health Care Ltd (Primary) collects the information on this form (Information) to process your feedback/complaint regarding Primary or one of the companies in the Primary group. If Primary does not collect the Information, Primary may not be able to receive or process your feedback/complaint.

2. Primary may disclose the Information to its related body corporates, but it is not anticipated that your Information will be shared with any entity outside the Primary group nor that it will be disclosed to any overseas recipients.
